Vaccinations are a safe and effective way to protect your child from serious illnesses, By keeping up to date with their routine immunisations, you are helping to safeguard not only your child;s health but also the wider community. The practice follows the national policy for childhood immunisation. Further information can be found on the NHS Immunisation Schedule website.
Please note if someone else (grandparent, nanny, relative) is bringing your child for his/her appointment, nurse cannot administer vaccinations without a Consent Letter from the parent.
Childhood Immunisations Disclaimer Form
We strongly recommend routine childhood vaccinations to help protect your child from serious illnesses, In the rare event that you choose not to vaccinate your child, we ask you to complete a childhood immunisation disclaimer form. This ensures your decision is recorded appropriately.
