We are pleased to accept registration requests from patients who are living within the area indicated on the map below, but we do also offer Out of Area Registration.
All Registration documentation can be downloaded and printed below, we would also ask you to review our Practice Behaviour Agreement . Then simply complete the GMS1 form and the relevant Health Questionnaires, and bring them into the surgery to finalise your registration with Parkshot Medical. We accept new registrations any time after 12noon. It can take up to 7 days to process and confirm Registrations.
For GP Practice and Out of Area Registrations you will need to complete and submit both, the GSM1 GP Practice and Adult or Child Questionnaires (all children under 16 ), plus the appropriate Out of Area form, if applicable. Please note that we cannot accept new patient registrations until all the necessary forms are completed.

Please note that our Surgery Health Visitor is notified when any child under 5 years old moves into our catchment area.
If you are visiting the area and require immediate treatment, you can register as a temporary patient. Please visit Parkshot Medical Practice in person to complete a temporary registration form, or you can download our temporary partient registration form and then bring this with you.
You can be registered as a temporary patient for up to three months.
This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you can, if necessary re-register as a temporary patient or register permanently with us if you are remaining in the area.
