Travel Risk Assessments

Travelling abroad?   You must complete the form below, so that our nurses can determine what vaccinations are required.   It is important to inform us as early as possible, preferably at least 6 weeks before you travel.

 

* Please print the Travel Risk Assessment form below and bring it to the surgery in person.  Please also be aware we do not accept or process any travel forms sent to us via email. When you bring your form into the surgery, the patient liaison team will check the assessment form and book you a double appointment with our Matron.

Vaccines may have to be ordered, as they are not always kept in stock.   Usually a second appointment is required, and this typically needs to be at least 2 weeks before you travel.   The majority of travel vaccinations require private prescriptions and incur a charge, as they are not provided by the NHS.
