HOW TO REGISTER
 

Welcome to Parkshot Medical Practice

We are pleased to accept registration requests from patients who are living within the area indicated on the map below.

Please note that the map below is a window into another website which we have no control over. You may find, particularly on mobile devices, that you may need to drag the map left to enter details and click.

Please call into the Surgery and talk to any member of our Patient Liaison team to register.  You will need to provide us with photo ID ( ie passport, driving licence ), proof of address and your NHS number, if you have previously registered with a NHS GP Practice.

We can only take new registrations before 6.00 pm on weekdays and up to 11.30 am on Saturdays.

PLEASE NOTE: YOU NEED TO BRING YOUR COMPLETED REGISTRATION FORM, QUESTIOANNAIRE, PHOTO ID AND PROOF OF ADDRESS TO THE SURGERY IN PERSON.  WE DO NOT ACCEPT REGISTRATIONS VIA POST OR EMAIL.

Do you live Outside of Our Catchment Area?

 

We can register you as an Out of Area patient.  As an Out of Area patient, we are not able to offer home visits.

If you are already a patient but moved out of area or a new patient, please complete the required forms below. As an addition new patients need to fill in a New Patient Questionnaire too please.

Please note that patients are often removed from our list by the Primary Care Support Service, if contact details are not kept up to date. Do make sure that we are informed of any change of address or telephone number as soon as possible.

 

Medication Information

Please note that Parkshot Medical Richmond does NOT routinely prescribe BENZODIAZEPAM's ( e.g Diazepam, Lorazepam & Temazepam,ect ). If you are taking this medication when joining the practice, you WILL be asked to see our phamacist with a view to reducing and eventually STOPPING your medication.

Temporary Patient Registration

If you are visiting the area and require immediate and necessary treatment, you will need to register as a temporary patient. Please visit Parkshot Medical Practice in person to complete a temporary registration form, or you can click here to download the form which you will need to print, complete and bring to our reception.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
